A medical alarm is integral to any Falls Management Program. Lifeline has designed the following self-assessment tool to help identify patients who are at-risk for falls. Patients can fill the survey out themselves or with the assistance of a healthcare professional or caregiver.

Overcoming denial of need is very important and the 'Is it Time for a Medical Alarm' survey can help! Lifeline has created a handy self-assessment tool to help seniors, caregivers or those with chronic medical conditions assess their level of need for a personal response service. After answering the following questions and adding up points, they can refer to the scoring scale to determine their level of need. You can also find the survey on the Lifeline public website where potential subscribers can fill in the survey and score themselves.
